Although each patient presents a unique set of challenges, our experienced nurses are able to anticipate the medical needs of our patients and create a personalized experience. DHHC has passionate and caring Registered Nurses (RNs) and Licensed Practical Nurses (LPNs), who work with your doctor to provide home care services.

Our trained Nurses provide the following services:

  • The assessment, skilled treatment, and support of DHHC nurses provide the high-quality care you need to recover at home.

  • Diabetic Training, Respiratory Care, Rehabilitative Care, Supervision of Medications, Treatments, and Injections, Wound Care, Family-Patient Health Teaching, Lab Work, and Dressing Changes.

  • Educate and manage your medical schedule, and ensure that you are fully aware of and familiar with your diagnosis and the medical plan in place to get and stay healthy.

  • Our Nurses are part of a diverse team and are fluent in many languages like Portuguese, Spanish, Haitian Creole, and French. 

 

Home Health Aides

A recently diagnosed condition or a chronic illness can be life-changing and stressful. In some cases, basic tasks can become difficult and overwhelming to you and your loved ones. Let our Homecare staff help you overcome those difficult times. DHHC Home Aides are trained to assist our patients with patience, dedication, and understanding.  DHHC Home Aides assist patients with the following tasks: 

·       Bathing and dressing of elderly

·       Convalescents and disabled patients

·       Vital signs monitoring 

·       Monitor and report the patient's physical, mental and emotional state

·       Provide Emotional support

·       Light Housekeeping

·       Cooking

·       and more.. 

In-Home Rehabilitation

DHHC staff will work with you, your family, and physician to put together a plan specifically tailored to meet your unique needs and circumstances

PHYSICAL THERAPISTS:

We will come to your home to provide physical rehabilitative therapy such as exercise, weight training, and instruction on the use of equipment.

OCCUPATIONAL THERAPIST:

Our therapists are fully committed to getting people back to their productive selves and thus provide for detailed, high-quality services addressing patients’ physical, mental or cognitive diagnoses. Getting back to work isn’t just vital to one’s livelihood, but hugely imperative toward an individual’s peace of mind.
